Data Protection Privacy Notice – Susan Grainger Hypnotherapy
The personal information you have provided when beginning a course of therapy sessions will only be used by Susan Grainger as data controller and processor, in accordance with General Data Protection Regulation 2018.
The personal data you require is your name, home address, email address and date of birth. Details of your family members, employment status and GP surgery. This information is kept securely in a locked filing cabinet in the office.
These details are kept confidential and shared only in a safeguarding situation when they may be shared with the Police, Social Care or a medical professional in order to keep you safe. This is a statutory duty of care. Unidentifiable material may be used in case studies.
All clinical notes will be retained for 7 years AFTER the end of therapy in accordance with Holistic Insurance company guidelines. After that said retention period, they will be disposed of as confidential waste.
The GDPR provides the following rights for individuals:

- The right to erasure
- The right to restrict processing
- The right to data portability
- The right to object
- Rights in relation to automated decision making and profiling

What personal data we collect and why we collect it
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognise and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
